Your child can be referred
to the Children's Development Center for the following services
- Neuropsychological Evaluation
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Inclusive Full Day Pre-School Program beginning
at age 2 years 7 months
- Autism Program beginning at age 2
Who can make the referral?
- A parent or family member
- A family physician, pediatrician or other healthcare
provider
- Case managers, social workers or mental health
providers
- Counselor, teachers
Who should I contact at the Children's Development
Center?
Contact our Case Manager at 326-0565 x 101, and
let the case manager know you are referring a child for a service.
The case manager will then provide direction and explain the process
for obtaining services.
To finalize the referral process, you will need
to provide:
- Your child's date of birth
- Child's social security number
- Name of child's primary care physician
- Insurance information
Please have this information available when you
contact the Children's Development Center. Once we receive the information,
we will obtain the necessary authorizations and prescriptions. A
scheduler will then contact you, as soon as possible (within 30
days) for an initial evaluation.
What if my child needs additional therapies?
If your child is currently receiving therapy
at the Children's Development Center, and you would like to discuss
the possibility of scheduling an evaluation for additional therapy,
please contact our Case Manager at 326-0565 x 101.
